Main causes of the incident was too much exposure to light your aquarium. Cyanobacteria contain chlorophyll a, make the process of the light in their food through photosynthesis. So evolution and getting really fast light there. High levels of phosphorus and other nutrients in the water can also cause of the outbreak. It is also possible due to the introduction of some recent, non-isolated pot plants. Of tank dirty and unclean water is nothing but the main concern.
